3/23/2023 EnchantaVaughn Approved Behavior: Client verbalizes positive self-talk and affirmations around his family at the start of the session, as witnessed by QMHP upon entering the home. QMHP engaged mother in a discussion to determine behaviors since the last session. Mother reports client struggles with managing her anger and she will lash out in the home and throw things at her siblings when she cannot have her needs met. Behavior: Client utilizes deep breathing skills and different coping strategies to stay calm while discussing her service objectives and treatment plan. Client openly discussed her feelings about her current state and how she feels. Client was able to discuss with QMHP on how she can be a better child by listening to her mother more often. Intervention: QMHP held consultations with client and mother about effective intervention strategies that build on the client's strengths. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP encouraged clientâs mother to provide frequent positive reinforcement to maintain client's interest and motivation in maintaining positive home behaviors and completing her individual home tasks. Intervention: QMHP monitored client individualized behaviors in collaboration with todayâs lesson. QMHP encouraged client to continue making good choices and to utilize self-calming techniques (i.e. counting to 10 and deep breathing) when she finds herself losing patience or feeling frustrated. QMHP processed with her about her weekly behaviors, the good and bad, as well as, expectations. QMHP provided positive reinforcement when client was able to achieve her goals and objectives. Response: Client displays good social behavior throughout the session, as observed and supported by mental health professional. Client was able to discuss with QMHP on how she can be a better child by listening to her mother more often so that she will not get in trouble. Response: Client reflected on her weekly plans. Client stated she didn't need help with anything at the moment. Client communicated by smiling and exhibiting good eye contact. Client was able to discuss with QMHP on how she will learn how to talk to her mother about things bothering her in the home when it comes to her siblings.
